In Elden Ring he is found at the Murkwater Cave in Limgrave, ambushing the Tarnished, then begging for mercy when defeated, then promptly trying to ambush them again. He is sworn this time to Volcano Manor : Tanith uses him as a procurer of unhappy bargains. He sells items, accepts contracts, lies about what's behind any given door. He is unkillable across most of his arc, the canon takes care to revive him every time the Tarnished slays him.

The bell bearing is the only proof of his death, and even it is ambiguous : Patches has died many times across many games and his flesh keeps producing new merchant inventory. In the Mountains he sells St. Trina's Torch. He probably knows where Miquella is. He will never tell.
